Step 1: Have your controller or corresponding personnel complete the ELT form, found by clicking the link, and mail it to the Kentucky Transportation Cabinet, alongside the application fee of $150. The mailing address can be found on the form. For questions, you can email kytc.kyelt@ky.gov.
